Reporting Procedures for CYSA games
- Collect player (or coach) pass, game card
- File a Red Card Report in MySoccerLeague
- Write up a SendOff Report and mail to Discipline Chair of the league that the game was played under within 24 hours
- Email a copy of the SendOff Report to [email protected]
- Be sure and give proper detail of the incident on the report form.
- Make copies of all documents for your records.
- It is the coaches responsibility to secure the player pass from the disciplinary chair.

Reporting Procedures for Norcal games
- DO NOT collect player (or coach) pass
- Fill out Norcal Send Off Report via http://www.norcalreferees.com/
- It will generate a pdf and email to you, forward the pdf to [email protected]
- Be sure and give proper detail of the incident on the report form.
- It is the home team responsibility to notify the League Office of the send off
